Merakey is looking for a Supportive Living Worker to join our Carlisle PA, Behavioral Health Team

POSITION OVERVIEW

• Provide services and ongoing assessment to assigned supportive living consumers but not limited to consumer training and monitoring of skills in nutrition, housekeeping budgeting, hygiene, self -care, home safety, transportations, social interaction, health care, and vocational/educational

• Evaluate consumer's strengths and needs using a psychosocial evaluation and other assessment instruments which may be

• Develop service plans with consumers which include active participation of consumers and significant others, as appropriate, with the goal to increase independent living Actively use service plans in working with consumers.

• Appropriately maintains clinical records for assigned caseload, including all related treatment forms, correspondences, and logs, in accordance with state, county, and Merakey Behavioral Health policies and regulations and supervisor

• Regularly documents consumer's progress or lack thereof and activities in accordance with their service

• Transport consumers to appointments and other necessary

• Communicate regularly with other service providers involved with consumer, including family members of consumers, as appropriate, to facilitate provision of effective care.

• Handle consumer crisis in professional and effective manner, which minimizes harm and facilitates sportive

• Link consumers with needed services and encourage involvement in community-based activities in accordance with their treatment planning, and monitoring

• Adheres to all Merakey Behavioral Health, county, state, and Federal policies and regulations related to mental health treatment

• Complies with Merakey Behavioral Health ethics codes of conduct, as well as the ethics and code of conduct of own discipline, in all contact with clients, co-workers, and the

• Participates in regular clinical supervision and continuing education activities to further develop professional skills in mental health

• Participates in The Merakey Behavioral Health Quality Improvement/Quality Assurance

• Regularly attends department staff meeting and Merakey Behavioral Health staff

• Maintains client confidentiality in accordance with Merakey Behavioral Health polices
