The Staff Nurse will be responsible for:

• Ensuring that ambulatory patients are seen in a timely manner by the appropriate clinician(s) within the Department of Neurology, and that appropriate coordination of care and communication is maintained with the patients and referring providers.
• Providing clinical telephone triage, especially for urgent clinical situations
• Reviewing and triaging of non-urgent referrals from non-Neurologists to determine best fit for scheduling.
• Triaging care to the appropriate specialist and/or disease-specific discipline for patients and/or referring providers who call seeking care in the department of Neurology.
• Collaborating in the management of the outpatient referral system and providing clinical triage and direction to the most appropriate provider, within the appropriate timeframe.
• Reviewing collected medical information and informing call center staff if additional records are needed.
• Acting as a clinical liaison for issue resolution within the call center and between the call center and BWH providers/referring providers, in collaboration with department leadership.
• Leading basic education within the call center around Neurological disease, with the goal of ensuring exemplary customer service and optimization of the Brigham Experience.
• Maintaining familiarity with department activity and a close working relationship with clinicians and support staff to provide the most appropriate triage and facilitate timely and effective communication and scheduling.
• Participating in departmental and organizational quality management and/or quality improvement projects as directed by the leadership team.
• Proficient use of the electronic medical record, documenting in a manner that is clear, complete, concise, and in SBAR format when appropriate to remain compliant with nursing standards.
• Working in a normal office setting; work entails extended periods of sitting and fast-paced, phone-based care.
• Prepares, performs, and documents the coordination of clinical care including, but not limited to, assisting patients with medication questions, delivering lab results, and providing patient education as needed and requested.
• Provides cross coverage for other Access RNs within the Call Center
• Other duties as assigned by the supervising physician or leadership team.

Qualifications
To qualify you must have:

• Education: Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required for newly licensed nurses and external applicants.

• Experience:
  • Minimum of 2 years of recent (within 12 months) clinical nursing experience in an Ambulatory Clinic/Practice setting and/or emergency room setting required. Experience only in an ambulatory procedural area not sufficient.
  • Minimum of 2 years of Neurology and/or related experience required.
  • Experience with telephone triage and telephone-based clinical assessment techniques required. This is the safe, effective, and appropriate disposition of health-related problems and includes: assessing severity of symptoms, communicating urgent patient needs to the LIP, referral to appropriate setting or level of care, documenting interventions and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergency situations, and assuring continuity of care.
  • Experience working in a Call Center or similar area and handling of referrals strongly preferred.
  • Experience in communications and customer service strongly preferred.
  • Case Management experience preferred.
  • Experience with team-based care preferred.
• Other:
  • Nursing certification in Ambulatory Care or ambulatory specialty area preferred.
  • Experience with EPIC preferred.
