CORE JOB SUMMARY
The Senior Case Worker serves as the primary development resource for families and delivers direct social services to families. The incumbent works in close collaboration with medical doctors and other members of the health team to further their understanding of significant social, emotional, and developmental factors affecting families.

Travels to homes, daycares and other natural environment locations as necessary to conduct the Individual Family Support Plan meetings Assists parents of eligible children in gaining access to early intervention services as well as additional support services available in the community. Provides education to families of eligible children on the services available in the community to enhance child developmental activities. Collaborates with personnel in community-based agencies to develop appropriate intervention strategies for eligible children and their children. Facilitate authorization of appropriate level of services for each child and family enrolled in the program. Documents services provided in the prescribed manner and completes paperwork prescribed by the Department to include documents, such as the Individual Family Support Plan (IFSP) requested by the state, etc. Completes required paperwork as needed for input into data and billing systems.

Monitors outcomes, data collection, and record keeping. Leads weekly group supervision meetings for staff. Meets monthly data and budgetary reporting requirements. Participates in visits with physicians during scheduled office hours or by appointment. Assesses developmental progress at home and office visits and promotes knowledge of development progress. Maintains a resource and referral file to assist families in finding timely and appropriate community-based programs and resources. Scores and interprets developmental assessments. Maximizes the continuity of care provided to enhance developmental potential. Maintains ongoing communications with family members to capitalize on service effectiveness. Participates as a member of an interdisciplinary team while serving as the single point of contact for families in need of early intervention services. Ensures the provision of early intervention services in a cost-efficient manner. Contributes to the development of a family support plan and monitors its implementation and modification. Coordinates activities with other case-related medical and non-medical personnel. Tracks services provided to each family. Adheres to University and unit-level policies and procedures and safeguards University assets.
CORE QUALIFICATIONS
Education:
Bachelor's Degree or higher from an accredited university with an emphasis in the areas of psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education
Experience:
Minimum 3 years of relevant professional experience with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management
