As an Accolade Social Worker, you will be a trusted advisor who helps our members by identifying, assessing, planning, coordinating, monitoring, and evaluating social services throughout the continuum of care to meet their health care needs. Your primary focus will be on identifying the member's psychosocial needs and barriers to care. As an expert in advocacy, you will have the opportunity to connect members to resources available at the health plan, employer, and community where the member works and/or resides to ensure they get the right care at the right time.

A day in the life...

  • Assumes a consultative role with all internal interdisciplinary care teams which is led by a registered nurse to achieve optimal clinical and resource outcomes.
  • Serves as a team member on a multidisciplinary care team responsible for monitoring and assessing members' social needs; seeks to meet the member where they are to promote trust and increase self-efficacy; understands diagnoses and procedures to ensure high quality and cost efficiency of care and service.
  • Performs a comprehensive psychosocial assessment of the member's health status, education, and level of support needs. The evaluation includes condition-specific issues, barriers to care, mental health status, cognitive functioning, life planning activities, cultural and linguistic needs, caregiver support, and resources.
  • Develops a holistic care plan which includes long and short-term goals, a self-management plan tailored to members' needs, and identification of barriers to meeting goals or compliance to plan of care.
  • Performs ongoing monitoring and management of members including scheduled follow-up with members, discussion of the plan with members, appropriate services/education to address needs, and appropriate referrals with supporting documentation to the case manager, medical director, pharmacist, behavioral health clinician, community resources, and providers; assessment of progress towards goals, modification of plan/goals as needed.
  • Formulates and implements policies and procedures for the organization and operation of social work activities in partnership with clinical readiness and quality teams.
  • Develops and manages social work education resources appropriate for use by internal customers.
  • Plans and implements in-service training programs on various social work topics.
  • Effectively manages multiple priorities, initiatives, and projects.

What we are looking for...

Required
  • Master's Degree in Social Work
  • Experience in identifying, planning, and managing member care with a minimum of three years of current experience as a medical social worker in a health plan or community-based setting.

Preferred
  • Demonstrated knowledge of commercial/employee populations
  • Ability to understand and communicate client's medical benefits, claims, and care coordination with a focus on advocacy and effective utilization
  • Strong communication skills, including the ability to actively listen and engage with clients telephonically
  • Experience with real-time electronic health record documentation
  • Self-discipline and ability to prioritize professional responsibilities in a work-from-home environment
  • Desire to continue clinical education to ensure the practice is at the top of licensure.
