The main focus of this position is to provide nursing services and to meet the identified health care needs of Help At Home clients. The RN works within the framework of the agency's policies and procedures to ensure compliance with all applicable state, federal and local administrative rules and regulations, as well as accreditation standards concerning the health care needs of the agency's consumers. This position reports directly to the Residential Director.

We are currently seeking Registered Nurses who have availability to work day, evening, night or weekend shifts.

Responsibilities
  • Provide supportive services to individuals
  • Collect data, as assigned, and report pertinent condition changes to the QIDP
  • Report responses to supportive care within the specified work day and report to QIDP about information that would impact the services and/or the Individual Support Plan.
  • Demonstrate the ability to care for a variety of individuals and report issues to the QIDP that might indicate a case conference is needed
  • Participate in case conferences, as requested
  • Follow Individual Support Plan and track goals
  • Document findings accurately, in a timely manner and on appropriate forms, according to agency policies and procedures
  • Provide accurate and timely timesheets
  • Follow assigned schedule, as assigned by agency or notify QIDP when schedule cannot be followed
  • Provide for the individual's daily care:
    • Bathing, dressing, and grooming
    • Eating
    • Elimination
    • Transferring and ambulating
    • Creating a positive home environment, developing trust with clients and families, and offering emotional support to clients and their families
    • Communicate and actively listen
  • Determine measurements accurately (temperature, pulse, respirations, blood pressure, weight, intake, output, and food consumption (if in High-risk plan)
  • Observe and report any changes in the consumer's appearance, behavior, and/or mood to the QIDP
  • Deliver medication, as required

Minimum Qualifications
  • Completion of a state-approved RN license
  • Must be in good standing with the state aide registry, if currently required to be entered on that registry
  • Knowledge and competency performing supportive service procedures
  • Ability to follow care plans
  • Recognition of the needs and concerns of others and the ability to maintain constructive working relationships
  • Must have a valid Illinois motor vehicle operator's license, access to a reliable vehicle and current automobile insurance coverage.
  • Must be willing to accept assignments without regard to location, socioeconomic environment, disease, ethnic origin or religion
  • Must be willing to work on weekends or evenings, when assigned
