Description:
The heart transplant coordinator provides heart failure nursing care and post-transplant follow up and nursing care to patients. Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. Provides education to the recipient regarding the transplant process as well as long-term post-transplant care. As a contributing member of a health care team, collaborates with other members of the health care team to promote effective patient care. Performs required documentation and communicates in a competent and effective manner to meet patient and/or family needs as well as regulatory requirements. Recognizes and consistently applies knowledge of Loyola's 14 Magnet Forces.

  • Coordinates the clinical care of the heart failure or heart transplant patient during all phases of transplant. Reviews diagnostic tests, laboratory work, and other pertinent information.
  • Assists in medical and medication management working within his/her scope and at the direction of a physician.
  • Educates and answers any questions or concerns of the patient and/or family on all aspects of heart failure and the transplant process.
  • Prepares and submits information to UNOS,CMS,JCAHO, Organ Exchange programs, insurance companies, etc. related to transplant service inquiries as needed by the department.
  • Performs administrative functions as preparation of QI reports, attending meetings and conferences, etc. as needed.

Position Requirements:

Minimum Education Required: Bachelors Degree

Specify Degree(s): Nursing

Minimum Experience Required: 1-2 years of previous job-related experience

Preferred: 3-5 years of previous job-related experience

Licensure/Certifications Required:

CPR (Cardiopulmonary Resuscitation)

Current Registered Nurse License State of Illinois

Preferred:

Specialty certification in clinical or functional area of nursing granted by a national nursing organization.
