Trinity Health Of New England is looking for an experienced Inpatient Care Coordinator to join Mercy Medical Center and our team of mission-driven healthcare innovators. This person is responsible for assisting physicians while providing exceptional treatment to patients with a variety of medical conditions.

This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.

You Will Be Responsible For

Exceptional Patient Care: Utilizes the nursing process and collaborates with the other health care disciplines to deliver holistic patient care to patients of all ages.
Excellent Analyst: Collects comprehensive data pertinent to the patient's health or the situation. Analyzes the assessment data to determine the diagnoses or issues. Identifies expected outcomes for a plan individualized to the patient or the situation.
Continued Professional Growth: In collaboration with Care Coordination Manager, you are responsible for professional development of Discharge Planners and Care Coordination Assistants.
Demonstrates Clinical Competency: Within 24 hours of admission, initiates discharge planning assessment by interviewing the patient and or family to determine the patient's discharge needs and documenting the assessment. Delegates patients and families with complex psychosocial, financial and legal needs to the discharge planner. Coordinates the plan and provides oversight to the Discharge Planner.
Education: Graduate of an accredited School of Nursing. BSN Required, Master's preferred.
Licensure: Current Licensure as a registered nurse (RN) in Massachusetts.
Certification: CPR certification required. CCM or ACM preferred.
- Possesses knowledge of budget processes, quality improvement, report distribution, TJC standards, state and federal regulations, six sigma and LEAN methodologies.
- Basic computer skills required.
- Effective communication, organizational and critical thinking skills.
About Trinity Health Of New England:
Education: Bachelor of Science in Nursing (BSN) degree required, Master's preferred.
Experience: 3-5 years in Case Management, Utilization review, or similar experience preferred. Hospital or community-based experience preferred.
