Job Role Summary

The Prescription for Hope Violence Intervention Specialist is responsible for providing comprehensive support services through wrap-around case management which assists patients, clients and their families to find practical ways of overcoming social, emotional and environmental barriers to optimum functioning and attainment of health.

Essential Functions and Responsibilities

  • Responsible for providing comprehensive support services through wrap-around case management which assists patients, clients and their families to find practical ways of overcoming social, emotional and environmental barriers to optimum functioning and attainment of health
  • Acts as mentor and advocate. Coordinates wrap-around case management services with other Prescription for Hope team members and community agencies
  • Provides therapeutic intervention to individuals and families, ranging from assessment, crisis intervention, community resource referrals, patient/family advocacy, support, outreach, and follow-up
  • Provides education and training to introduce or improve coping skills of individuals and families
  • Documents interventions and services in designated records
  • Utilizes problem-solving skills to identity and facilitate problem resolution in team functioning
  • Participates in staff meetings and education programs for staff and community agencies as assigned
  • Assumes responsibility for continuous professional self-development
  • Alters activities/behaviors to reflect/ensure adequate care appropriate to the age of patients served (i.e. adolescent, geriatric, pediatric, neo-natal, adult) described in the area/unit age-specific and/or job specific competencies
  • Collaborates with multi-disciplinary team members to facilitate the coordination and delivery of services
  • Initiates advocacy/liaison role for patients/families in referrals to and from community agencies: collaborates with agency and hospital personnel in referral activities: cultivates referral resources
  • Interprets impact on client functioning of environmental conditions and family interactions through outpatient encounters and potential home visits

Job Requirements

  • High school diploma with a minimum of two years of direct community experience and/or coordinating wrap-around services
  • Bachelor's degree in social work, nursing, addiction, education, counseling, psychology, sociology, criminal justice or related field preferred
  • Experience in a healthcare or community setting
  • Demonstrated knowledge of community resources
  • Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements

  • Prioritizes tasks and works effectively with complex tasks under time pressures
  • Communicates in a concise and easily understandable manner with professionals, paraprofessionals, and the target population

  • Ability to be nonjudgmental and to mentor and encourage strength based approach to patient/client services

  • Ability to meet with patients and families and to develop a trust level necessary to provide services and to encourage positive outcomes
  • Acts as a patient advocate
  • Knowledge and skills necessary to provide care appropriate to the age of patients served. Must demonstrate knowledge of growth and development principles over the life span and possess the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age-specific needs. Ability to use age appropriate communication and skills detailed in the department/area/unit job specific/age specific competencies
  • Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, initiative
  • Ability to perform duties independently under the direction of specified supervisor
  • Ability to recognize own limitations and seek assistance from appropriate resources as needed
