Purpose

To provide extended case management services to particularly vulnerable refugee clients. The position will work with a dynamic team to provide services to ORR-eligible population. The Case Manager will work primarily with clients who have medical needs, housing, mental health issues, or any other barrier that makes it too challenging for them to become self-sufficient during or after the initial resettlement period of 90 days.

Responsibilities

  • Coordinate services with the Reception and Placement, Matching Grant, and other departments' case management teams to determine new arrival case enrollment into the Preferred Communities program based on specific need for intensive case management services.
  • Creates comprehensive case plan with client and works together with Resettlement Program Case Manager and employment team to assist client to attain self-sufficiency.
  • Maintain electronic and physical case files complaint with the program's guidelines.
  • Connect client to in-house support and make referrals to external service providers such ESL, transportation, immigration services, and childcare.
  • Arrange appointments for medical visits, social security office, school enrollments, legal services, etc.
  • Aid with filing for disability, food stamps, FMLA and other services.
  • Perform outreach and connect with community organizations as needed based on clients' medical conditions.
  • Assist clients with insurance applications, provide health care and medical system orientation.
  • Coordinate Group-Only Programming.
  • Assess the need for client direct assistance, distribute and keep record of it.
  • Community partner/service provider training and outreach to increase community capacity to serve vulnerable refugees - medical/mental health providers, human services agencies, social services, landlords, legal service providers, etc.
  • Provide refugee community member training.
  • Stays abreast of refugee and immigrant issues, and available services to refugees, using current information to better identify and serve refugee clients.
  • Must have reliable transportation and a valid driver's license.
  • Perform other related duties as requested by the Resettlement PC Senior Case Manager, the Director of Resettlement and/or his/her designee.

Qualifications

Education: Associate degree or 3 years of relative work experience.

Experience: Previous work experience with refugee resettlement case management or other direct social services and excellent interviewing skills.

Other Skills:

Computer literacy, using word processing and database applications. Strong English writing skills. Spanish language fluency required.

Special Requirements

  • Must have a valid driver's license.
  • Must complete motor vehicle record check.
  • Must be willing and able to travel.
  • Background check required.
