GENERAL SUMMARY/ OVERVIEW STATEMENT: .

The physician assistant (PA) will be an integral part of the Division of Infectious Diseases (ID) Immunocompromised team's approach to evaluating and treating patients within the inpatient and ambulatory areas of BWH. Under the direction of the supervising physician, the PA will be involved in all aspects of patient care. The PA will also be responsible for liaising with our reciprocal surgical and medicine services and triaging patient issues/concerns to faculty. In addition to clinical skills, this person should possess strong leadership and communication skills and be able to work effectively with other members of the team including physicians, fellows, and staff.

The ID Transplant Physician Assistant must demonstrate competency to effectively diagnose, formulate appropriate treatment plans, and efficiently implement a care plan under the direction of the supervising physician within the context of the following areas:

• General infectious disease conditions including, but not limited to bacterial, viral, fungal and parasitic disease frequently seen in an outpatient clinic or inpatient settings.

• Understand the complex medical environments of the immunocompromised patients we care for at BWH including, but not limited to, those with: advanced organ dysfunction (end stage heart, lung, and/or kidney disease); advanced malignancy; a solid organ transplant (SOT) or a hematologic stem cell transplant (HSCT); and indwelling hardware and medical devices.

• Appreciate complex medication interactions.

• Understanding the limits of current diagnostic and therapeutic strategies for infections in these high risk patient populations and when novel strategies are needed such as investigational approaches and the use of salvage antimicrobial therapies.

• Understand the risks, benefits and alternative antimicrobial strategies for key pathogens for the immunocompromised patient such as CMV, PCP, and Aspergillus

• Indications and interpretation of radiology, pathology and laboratory results

The PA will be responsible for:

• Obtaining and coordinating the following general items: history and physical examination, laboratory results, charting of clinical findings in the EMR, orders, nursing instructions, and other required elements.

• Educating patients regarding a variety of Infectious Diseases issues associated with solid organ transplant.

• Coordinating admissions and transfers to BWH (in collaboration with the supervising attending physician) and ensuring that informed consents are obtained as needed.

• Triaging patient calls and questions related to clinical care

• Acting as a liaison for referring physicians and the Immunocompromised ID group.

• Performing patient assessments in the outpatient ID Clinics at the BWH and/or DFCI and inpatient floor/ICU as needed. The PA will be responsible for ensuring appropriate diagnostic procedures occur in a timely manner though careful coordination with other services such as Interventional radiology and pulmonary, facilitating clinic appointments for patients requested to be seen by the ID service, communicating with patients regarding test results, completing prescription refill requests, and providing initial coverage for ID physicians who are unavailable.

• Collaborating with Immunocompromised ID Group and reciprocal services to improve patient access to our services, establish ambulatory goals for the delivery of care to patients, participate in clinical research, manage communications between physicians, patients, family, and staff. Attending inter-disciplinary meetings as needed to discuss patient care, policies, and procedures.

• Working collaboratively with team members, ensure an environment conducive to the well-being and safety of patients and staff, and continually update clinical knowledge and skills through consultation, formal and informal education and review of literature.

• Performing other clinical duties as assigned by the Supervising Physician.

Qualifications
QUALIFICATIONS:

  • The appropriate candidate will have successfully completed a graduate level physician assistant program (Masters degree) accredited by the Accreditation and Review Committee for Physician Assistants (ARC-PA) or such other accrediting agency as may qualify a graduate to sit for the NCCPA certifying examination
  • BS/MS joint programs and those candidates with a BS and PA certificate are eligible, but ONLY for those who graduated BEFORE 1995
  • Must have at least two years of experience practicing as a PA to qualify for the PA II position
  • Certification by the National Commission on Certification of Physician Assistants (NCCPA) must be obtained prior to first day of employment
  • Must hold a current valid license or be eligible for licensure as a physician assistant in the Commonwealth of Massachusetts
  • Must have or eligible for, and maintain, a current DEA registration and Massachusetts Controlled Substance Registration
  • Must complete 100 hours of CME according to national and state regulations during each two year period of employment
  • Must complete the BWH Professionalism Program within three months of date of hire
  • Must complete the BWH Procedural Sedation Training if deemed necessary

PA's:

  • Must exercise the highest degree of professional judgment under the direct supervision and guidance of a licensed physician.
  • Must maintain a professional attitude towards patients and exercise such mature independent originality in solving problems not covered by existing guidelines, within legal limits, to meet the needs of the patients.
  • Must maintain a professional attitude towards patients at all times and an excellent and collegial working relationship with other personnel involved in patient care.
  • Must demonstrate understanding of procedures, policies and documentation required to ensure compliance with hospital standards of care.

The PA shall demonstrate the following clinical knowledge, skills and competencies:
  • For the most common diagnoses admitted to (service), demonstrate competency to effectively diagnose, formulate appropriate treatment plan, and efficiently implement the plan without much direction, and with a greater level of independence, from the attending physician.
  • Clinical skills and knowledge as well as professionalism of the PA candidate are assessed by Supervising Physician.
  • Candidates with two or more years of relevant PA experience prior to joining the (service) become eligible for a preliminary evaluation of competencies approximately 3 months after joining the service. The purpose of the preliminary evaluation is to provide feedback to the candidate on progress towards level of competency required and provide guidance on additional development needs. After approximately 6 months, after joining the ID service, the candidate then becomes eligible to complete the assessment and, if successful advances to a PAII.
  • If a candidate does not pass the competency assessment, ID attending MDs will provide feedback on additional development needs. Re-assessment is possible after 6 months of focused development.
  • Competencies are assessed by ID Attendings either through direct observation or verbal communication. The attending physician(s) performing the assessment will fill out a competency assessment (designed by dept/division), attached. The candidate is responsible to provide example cases, which he or she has cared for patients with the most common diagnosis.

WORKING CONDITIONS:

PA is expected to perform duties and tasks as outlined above, in a fast paced, multi-specialty, inpatient and outpatient ambulatory setting. Noise, dust, chemical and temperature extremes will be consistent with the surrounding clinical environment at BWH.
