Description:
Provides pharmaceutical care for the outpatient populations according to professional standards and statutory requirements. Directs the functional activities for Trinity Health Detroit Pharmacy a closed system outpatient pharmacy. Directs the functional activities of technicians and interns. Provides medication preparation and distribution with a high degree of accuracy under strict time constraints. Develops safe, rational and cost-effective medication care plans to improve patient outcomes. Manages drug therapy and utilization across the continuum of care and monitors medication therapy outcomes. Works with personal assistant programs (PAP) to secure medications for low-income patients. Educates patients and providers. Engages in professional activities to improve the public's health awareness.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

1.Interprets and evaluates all prescriber's medication orders to ensure appropriate drug therapy for each patient in accordance with legal and regulatory policies, rules, and standards.
2.Reviews medication orders to determine appropriateness of therapy including, but not limited to, dose, dosage form, route of administration, schedule, starting time, and duration.
3.Monitors patient medication regimens for possible allergies, drug interactions, nutrient-drug interactions, contraindications, and potential adverse drug reactions.
4.Performs patient medication histories and counseling. Provides education materials, as required
5.Consults with and advises physicians and other health care professionals in the development of safe, rational, and cost-effective patient medication care plans.
6.Complies with all regulations governing the practice of pharmacy as defined by the State of Michigan, federal law, and other applicable governing agencies.
7.Directs and monitors pharmacy technicians, interns, aides, volunteers, and students in preparation, packaging, labeling, and distribution of medication.
8.Compounds and dispenses medications and injectables; recognizing that compounding and/or dispensing errors may result in significant patient morbidity and mortality.
9.Reviews and creates dispensing reports and accounting sheets.


10.Initiates, directs, coordinates, and performs specific departmental projects.
11.Adheres to and enforces the Formulary Medication System, and the departmental policies and procedures.
12.Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
13.Maintains the confidentiality of information acquired pertaining to patients, physicians, employees, and visitors at primary care center. Discusses patient information only among appropriate personnel in appropriately private places.
14.Behaves in accordance with the Mission, Vision and Values of the Health System.
15.Assumes responsibility for performance of job duties in the safest possible manner to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
16.Runs quality and accounting reports out of RX system.



OTHER FUNCTIONS AND RESPONSIBILITIES



Performs other duties as assigned.


REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
1.Education: Requires a Bachelor degree in Pharmacy. A Pharm.D or other related advanced degree is preferred.
2.Experience: Previous community and/or retail experience preferred
3.Licensure: A current State of Michigan Pharmacist license, or eligibility for licensure.


REQUIRED SKILLS AND ABILITIES

1. Broad knowledge of medications, compounding, and dispensing. Thorough knowledge of pathophysiology, pharmacokinetics, and therapeutics.
2.Interpersonal skills necessary to interact with patients, physicians, and other health care providers.
3.Analytical SMls necessary to organize and prioritize multiple job duties
4.General understanding of computers, including general operations and data retrieval
5.Communications skills necessary for clear written and oral communication.
6.Thorough knowledge of the regulatory, statutory, and professional aspects of pharmacy practice.
