Position Summary:

The Diabetes Education Coordinator is responsible for assessing the needs of the patient, family, and caregivers to develop individualized education plans for patients with diabetes at HCGH across settings and populations. Additionally, the Coordinator will be working in a multidisciplinary environment to assess the educational needs of hospital staff and develop education plans to enhance staff competencies.

Education:

BS degree in Nutrition or Nursing, or Master of Science in Nursing, from an accredited university.

Experience:

  • Minimum of one (1) year of experience as a clinical dietitian or registered nurse with patient education responsibilities.
  • Two (2) years of experience in diabetes management preferred.

Required Licensure, Certification:
  • Registered Dietitian (RD) and a State of Maryland Licensed Dietitian (LD) OR State of Maryland Licensed Registered Nurse (RN).
  • Certification as a Diabetes Care and Educator Specialist (DCES) or eligible for a Certification as a DCES.
  • Current accreditation by completing the required CEUs for the prescribed periods.

Knowledge/Skills:
  • Strong communication skills are necessary to interpret individual responses to assessments and treatment.
  • Excellent interpersonal skills to effectively communicate with patients, families, physicians,
  • and other health care providers both orally and in writing.
  • Ability to analyze historical, laboratory, and clinical data to formulate a comprehensive individualized diabetes education plan.
  • Proficiency in Microsoft Word, Microsoft Excel, and Zoom.
