Job Summary:

As a member of the multidisciplinary ACCS Integrated Team, the GLE Engagement Partner II provides day to day support to adults challenged with mental health issues who are living in a group living environment or a supported independent environment. The GLE Engagement Partner II delivers interventions to Persons that are clinically informed and are delivered in a way that is supported by evidence based practices. The GLE Engagement Partner II contributes to Bay Cove's mission by partnering with Persons to work towards personal goals inorder to achieve greater independence and improved quality of life. This is a non-exempt position.

Essential Functions of the Position:

  • Knowledgeably incorporates evidence based practices and philosophies into engagement strategies, treatment planning and the selection of interventions based on each Persons' assessed readiness and expressed interest in order to support long-term recovery. These practices include but are not limited to; Screening, Brief Intervention, and Referral for Treatment (SBIRT), Stages of Change, Motivational Interviewing, Harm Reduction, Seeking Safety and Trauma Recovery and Empowerment Model (TREM).
  • Participate on the Engagement Team of assigned persons as directed by the ACCS Clinical Supervisor and carry out case management activities for assigned individuals.
  • Implement interventions identified in Persons' treatment plans to assist Persons in developing and utilizing skills in the areas of activities of daily living; obtaining and maintaining housing, employment, school, and familial relationships; socialization and recreation; self-management of symptoms and medication self-administration; and health and well-being skills, such as healthy eating, physical activity, relaxation techniques.
  • Document interventions carried out, progress made towards goals, and observed changes to physical health and mental status in daily notes in order to better inform assessment and treatment planning.
  • Contribute to the provision of high-quality and effective clinical services and operational excellence by sharing observations in shift logs and by collaborating towards solutions during staff meetings.
  • Accompany Persons to medical, dental and psychiatric appointments or leisure activities via public transportation, agency vehicle, or staff car and teach skills for self-advocacy and safe use of community resources.
  • Collaborate with Care Coordination Entities and other providers to ensure access to appropriate medical, clinical, self-help, and peer modalities during times of crisis, care transitions, other life transitions and changes in health status.
  • Train Persons in medication self-administration procedures as designated in individual plans and administer medications in accordance with the standards of the Medication Administration Program (MAP) standards.
  • Keep abreast of, and implement, all safety plans and procedures to ensure overall health and safety of Persons including engaging in crisis intervention through de-escalation techniques and access of emergency services as needed.
  • Utilize recovery-oriented language and principles in all communication and interactions with Persons, colleagues, and collaterals.
  • Take on a specialized role based on interest, demonstrated skill, or expertise at the request of the Clinical Supervisor, Program Manager, or Assistant Program Manager (attach signed Specialized Role Description if assigned).
  • Perform other duties as assigned by supervisor.
Requirements for the Position:

  • Bachelor's Degree required, preferably in Psychology, Social Work, or a related area.
  • A High School diploma or GED plus three years direct care experience providing services to adults with mental health issues or related population can be substituted for the above degree requirement.
  • Demonstrated understanding of basic clinical concepts.
  • Demonstrated crisis assessment and crisis intervention skills.
  • Demonstrated skill in de-escalation and the provision of behavioral supports.
  • Demonstrated clinical writing and verbal presentation skills.
  • Proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps.
  • Department of Public Health certification in medication administration required within 3 months of hire.
  • Flexibility to work evenings and weekends on an as needed basis.
  • Ability to climb stairs and carry up to 40 pounds, as well as the physical capacity to assist people with mobility impairments as needed.
  • Valid driver's license preferred.
  • Use of personal cell phone for work purposes.
  • Bilingual candidates encouraged to apply. Pay differential offered for candidates with fluency in certain essential languages.
  • A COVID-19 vaccination is a requirement of the position. One COVID-19 shot is acceptable, contingent on the individual receiving the second shot within the allotted time frame.
