The MAT case manager will serve as a case manager for the DC Medication-Assisted Treatment (DC-MAT) program and provide recovery support services to participants of that program. In this capacity, the Case Manager will provide and advocate for effective recovery-based services to aid in creating a stable environment for individuals participating in the DC-MAT program who are recovering from an Opioid Use Disorder (OUD). The Case Manager brings a unique asset to the DC-MAT, providing intensive case management supports to supplement the recovery efforts of the Peer Coaches.

It is imperative for the Case Manager to have a set of competencies including knowledge of the Recovery process, recognizing signs and symptoms of substance use and mental health concerns, familiarity with community resources that aid independent living and the case management skills necessary to support participants. The MAT Case Manager should also have the ability to easily engage participants and build appropriate therapeutic alliances while maintaining solid boundaries.

RESPONSBILITIES:

  • Assist enrolling DC-MAT participants in the Peer Pathways program at VOACC, a peer support center located within the DC Hope Center along with DC-MAT. Working with the Peer Pathways Recovery Coaches, the DC-MAT case manager will:
  • Work one-on-one with each participant to determine strengths and needs to develop and meet self-directed recovery goals
  • Utilize tools such as Wellness Recovery Action Plan (WRAP) to help participants create their own individualized recovery plan.
  • Assist participants in determining the steps he/she needs to take in order to achieve his/her goals of self-directed recovery and housing stability.
  • Assist client in identifying and plugging into self-help groups, community groups, faith-based community groups to foster participants community involvement
  • Work closely with Program Director-MAT and Peer Pathways staff to collaborate continuity of care and place referrals to the community as needed.
  • Document and report concerns to Program Director in a timely manner.
  • Assist participants with empowerment skills to improve coping techniques and self-help strategies
  • Consult with direct supervisor or supervisor on duty regarding participants personal needs, behavior and service planning.
  • Follow appropriate critical incident protocol and accompanying documentation
  • Maintain a comprehensive and accurate written record of events that occur
  • As clinically appropriate, share their own recovery story and how they tackled obstacles or challenges in their recovery journey to help normalize participant experience.
  • Maintain healthy boundaries with program participants
  • Perform other duties as assigned by immediate supervisor or designee
EFFECTS ON END RESULTS:
  • Case sessions notes/documentation needs to be completed by 24 up to 48 hours
  • Must be able to conduct and complete intake assessments.
  • Must be able to facilitate/conduct two (2) group sessions with peer partners for a total of (8) per year.

  • The physical ability to travel to assigned locations and safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.
  • The physical to stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.
  • Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
  • Operating office equipment requiring continuous or repetitive hand/arm movements.
  • The ability to remain in a sitting position for extended periods of time.

REQUIREMENTS

  • Bachelors Degree in human services, social services or related field.
  • Min (1) years of experience in counselor and case management activities, including conducting assessments and case management.
  • Ability to communicate with persons served on weekend and holiday as need for continuation of care.
  • Working knowledge of Electronic Medical Records i.e. Foothold/Awards etc.
  • Demonstrated ability to assess and respond in crisis situations.
  • Ability to work both independently and as a member of a team
  • Strong problem solving and execution skills.
  • Strong PC skills in a Windows environment (i.e. Word, Excel, Power point and email)
  • Ability to use independent judgment and address conflict and find resolutions.
  • Excellent interpersonal, written, and oral communication skills.
  • Bilingual in any language a plus.
