$2,000 retention bonus when you work 24 or more hours a week.

Must work every other weekend.

Work a shift between the hoursof 7 am-9 pm Saturday and Sunday.

Work a shift between the hoursof 12 pm and 9 pm during the week.

We are currently hiring for CNAs to complete home care visits in our senior client's homes.You will use your vehicle to drive to senior citizens homes and you will assist them with tasks they can no longer do by themselves and use your training. We pay for your mileage and driving time in between home visits.

PRIMARY RELATIONSHIPS: Personal Care Service Manager, Personal Care Scheduler, Nursing Director, Home Care and Clinic staff, participants, family members and public.

OBJECTIVE: Under the direction and supervision of the Personal Care Service Manager responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of the home environment or the Adult Day Health Center as needed. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of Interdisciplinary Team.

PACE (Program of All-Inclusive Care for the Elderly) delivers all needed medical and supportive services to our participants.The program provides the entire spectrum of care and services to older adults with chronic care needs while maintaining their independence in their homes for as long as possible.

1. Consults each day with the Personal Care Service Manager and Personal Care Scheduler to be updated on Senior CommUnity Care participant's status, changes in plan of care and to confirm scheduled visits.

2. Under the direction and supervision of the Registered Nurse assists participants in the home with the following duties:
a. Personal hygiene such as skin, mouth, hair care and bathing.
b. Assists participants in and out of bed assisting with ambulation.
c. Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel.
d. Prepares meals, including therapeutic diets, assists participants with meals.
e. Assist participants to the bathroom or in using commodes, bedpans, or urinals.
f. Assisting participants with medications which are ordinarily self-administered.
g. Other duties taught by a health profession for a specific participant such as: taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.

3. Performs directed household services.

4. Reports changes in participant's condition and needs to the Personal Care Service Manager.

5. Completes appropriate documentation of services provided following Senior CommUnity Care policy.

6. Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

7. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

8. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

9. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.

10. Acts within the scope of his or her authority to practice.

11. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

12. Participates in and supports Quality Improvement Initiatives.

13. Performs other duties as required or requested.
