Job Description Summary:
Community Health Center, Inc. (CHC), with offices in Connecticut, Colorado and California, is one of the country's most creative and dynamic providers of primary medical, dental, and behavioral health services, and a leader in practice-based research, health professionals training, and use of innovative technologies to advance health and healthcare. CHC is designated as a federally qualified health center and a patient-centered medical home by HRSA, the Joint Commission, and NCQA, respectively. We deliver more than 600,000 patient visits per year from primary care hubs and community clinics across the state of CT, all connected by technology and common standards for quality. We employ several hundred medical, dental, and behavioral health providers who are engaged in practice, teaching, and research. Our Weitzman Institute is devoted to research and practice transformation and is recognized around the country as one of the premier research institutes focused on improving health care and health outcomes for special and vulnerable populations. In addition, the organization has developed three wholly owned subsidiaries from the original pilot developments within the Weitzman Institute: the National Nurse Practitioner Residency and Fellowship Training Consortium (NNPRFTC), the National Institute for Medical Assistant Advancement (NIMAA), and the ConferMed.

Job Description:

ROLES AND RESPONSIBILITIES:

Develop close working relationships with leadership and providers to ensure coordination of services related to HIV Prevention, HIV Treatment, STI Screening and Primary Care.

Responsible for recruitment, enrollment, and establishment of care who are in need of PrEP services and enroll patients as needed with all appropriate documentation.

Provide linkage to care and patient coordination for individuals seeking PrEP as a practical prevention tool.

Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care.

Complete screenings to identify readiness for PrEP and other barriers to care.

Assess benefits and insurance, and if needed, provide assistance to access payor programs.

Maintain appropriate and timely patient documentation of services utilizing electronic health records.

Work with the CKP Outreach Coordinator to implement and follow through of an outreach strategic plan.

Organize, promote and facilitate outreach events statewide to educate and raise awareness to the public about HIV risk and PrEP options.

Provide HIV and Hepatitis C testing to at-risk populations in the community, and at community events.

Conduct outreach to develop a network of community partners, develop MOU's as needed.

Enter HIV routine testing data for the Prevention grant in CDC SAMS efficiently and weekly. (Secure Access Management Services Portal)

Actively participate in CKP staff meetings, trainings, and any other meeting as directed by supervisor.

Special assignments, projects, and other duties as assigned by direct supervisor.

QUALIFICATIONS:

Associate degree in related field

Experience in healthcare settings, and community resources experience.

Computer skills using Microsoft software applications.

Effective teamwork orientation with the ability to communicate and build relationships

Effective communication skills, both oral and written, as well as excellent interpersonal skills.

Strong organizational skills, attention to detail, and demonstrate ability to manage multiple tasks, adapt to changing priorities and meet deadlines.

Understanding of and ability to access community resources.

Ability to work hands on with limited resources, independently or in a team setting.

Maturity, flexibility, compassion, and the ability to work well with vulnerable populations.

Preferred experience in providing crisis interventions, coordination of services, advocacy, and community outreach in working with individuals.

Energetic and willing to work flexible shifts including weekends.

Federally Qualified Health Center (FQHC) understanding a plus.

Must have a valid driver's license and a reliable vehicle a plus.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:

At times staff will enter tenement buildings and are required to climb up and down stairs. Most of the time staff is expected to be in the community. Travel time by foot or automobile is required.
