The Medication Assisted Treatment (MAT) Registered Nurse is responsible for providing patient care and care coordination for adults enrolled in the Certified Community Behavioral Health (CCBHC) program with opioid use disorder receiving medication assisted treatment. The MAT Registered Nurse supports integration of medical screening and services for Certified Community Behavioral Health Center (CCBHC) clients in collaboration with CCBHC providers and client Primary Care Providers. The Community Behavioral Health Nurse (CBH Nurse) practices at the highest level of licensure in the Community Behavioral Health Center service delivery model for the future, provides access to comprehensive and integrated client-centered care using evidence-based practice and assessment. The MAT Registered Nurse interfaces with key partners involved in the client's care including other Aspire CBHC team members, behavioral health professionals, primary care providers, and social services. The CBH Nurse works as part of a team providing innovative approaches to behavioral and population health management, engaging clients, families and providers in care planning and coordination, decreasing fragmentation of care for the client, to create a replicable model for increasing access and ensuring care is provided in the best and least acute setting for the client. This position will report to the CCBHC Medical Director in collaboration with the CCBHC Program Director will work closely with Behavioral Health Clinicians and the client care team to streamline an all-inclusive continuum of care. The MAT RN is responsible for assisting with initial intake and assessment as well as supporting and monitoring clients through all phases of treatment.
Schedule would be 20 hours per week and will include weekends.
