The Liberty Village program at The Home for Little Wanderers is a short-term group home program for up to 16 young men and women, ages 18-25 struggling with housing insecurity. The program helps these young people by providing a safe place to stay, along with clinical support, while more long-term housing options are identified in order to combat homelessness. The Case Worker ll will provide individual and family based therapeutic services, crisis intervention and case management for the Liberty Village program.

How You Will Be Making a Difference

Provide counseling and therapeutic support to clients.

  • Provide support to young adults to address a variety of issues which may include communication, parenting skills, substance use, domestic violence, trauma and housing needs.
  • Coordinate needed resources and services for young adults which include housing resources. Serve as an advocate for the young adults in order to support them in each of these areas.
  • Administer written assessments, summary reports, documentation, treatment planning, and safety planning to appropriate parties in a timely, well written manner. Submit reports, documentation and billing in a timely manner. Ensure that documentation meets utilization review and quality assurance standards.
  • Continuously communicate with client collaterals and providers of supportive services. This may include extensive telephone contact and scheduling and attending case conference meetings. Facilitate communication between various providers.
  • Plan, assess and provide all treatment and clinical interventions in an active co-therapy partnership w/ collaterals. Work as part of interdisciplinary team to provide quality services.
  • Attend staff meetings. Present client material, consult with supervisor and team members on challenging and difficult client issues. Participate in regular individual and group supervision. Attend clinical training and safety practice training.
  • May be required to provide support to families on a rotating schedule utilizing an on-call system Monday through Friday between 5pm and 9am and anytime on Saturday and Sunday.
  • Depending on the program, may be required to meet certain billing requirements on a weekly basis.

Qualifications
  • Bachelors Degree required in Social Work, Early Childhood Education, Criminal Justice, or Psychology (varies by program)
  • Experience with behavior management systems, treatment planning, crisis intervention, case management and providing individual, group and/or family therapy
  • Experience with rapid rehousing for young adults strongly preferred
  • Excellent verbal and written communication skill as well as experience working with DCF, DMH, DPH, DTA, MassHealth and other community-based organizations
  • Computer literate including Microsoft Office and Internet Explorer with the ability to learn new software applications
